1. You will need the following
forms and sources of information:
U.S. Swimming Registration forms to cover
each of your swimmers. This registration will cover your
swimmer from 9/1/07 - 12/31/08.
2. Completely fill out the
medical release form
3. Please review the
Winter
Fee Schedule and the
Fundraising
obligations.
4. Completely fill out the
registration form,
except for "Assigned Card #."
5. Fill out a
U.S. Swimming Registration form for each of your
swimmers. The Club Code is MAKO, and the Club name is the MIAMI
VALLEY AQUATIC CLUB. We will use this information to
electronically register your swimmer(s), saving you an additional
surcharge for paper work. We are sorry about the expense of these
registrations (none of it comes to us), but it is necessary from
both an insurance and competitive swimming standpoint. Your
swimmer can not compete in any swim meets until they are
registered.
6. Turn in all your
paperwork to a designated person at registration or to the
Richmond
mailbox at the northern or "Mako" end
of the spectator's gallery if you are pre-registering.
7. We will then do the
necessary paper and electronic work. At the end of registration
you'll make the initial payment, and establish
the remaining payment schedule.
Most fees are payable in three payments (Beginning of season, October
3, November
7).
A current USA swim card is required to swim in meets. The cost
for USA yearly registration (included in Bronze, Silver and Gold
fees) is $54. In addition, a $50 escrow account is requested for
swimmers competing in meets.
8. Once you've paid your initial
amount, we'll give you your Mako pass(es). These passes will
let your swimmer through the rec. center security check point to
access the pool. Do not loose this card. A
$20.00 fee is charged for any lost cards. These cards will
be turned in at the end of the season to Carol Eddy Dilek.
9. Parking passes are available at the Rec. Center
Information Desk. Identify your self as a Makos family.
